Go to your iPhone settings:
Settings / Accessibility / Touch / Call Audio Routing -- you'll see some choices:
- automatic
- bluetooth headset
- speaker
It may be possible that "speaker" is selected and not your headphones.
When I answer calls on my iPhone, I'm unable to hear the call in my headset.
